Appointment Request

Use this form to request an appointment with us. While we will do our best to accommodate your requested day and time, please note, you do not have an appointment until you get a confirmation from us!  A deposit for the office call charge of $65 will be charged at time of scheduling.  It is refundable if you call and cancel at least 24 hours before your appointment.
